Mrs Shearer, aged 75, was admitted to hospital after she suffered a fall at home. She sustained a right neck of femur fracture (fractured hip). Mrs Shearer lives alone with support from home care services and limited family intervention. While in hospital she was assessed by staff using the Specialist Multi-disciplinary Assessment Tool. This assessment highlighted that she was at a high risk of falls, had poor safety awareness and was nutritionally compromised. Mrs Sheerer was also deemed to lack capacity to make decisions in relation to her future care needs; therefore, a referral was made to the local Social Work Department to request a Community Care Assessment.
