During my two week clinical placement at Bankstown-Lidcombe hospital I was able to practice my documentation skills for observations of patients. At the time of documentation I was rushing and not thinking clearly because I had other patients to tend to. As the day progressed a Registered Nurse (RN) had called me over and told me I made a mistake whilst documenting. The mistake I made was filling the observations in the wrong column, which meant that the observations were confusing to read and inaccurate. This experience still resinates with me as I know how to document correctly and I could have avoided this mistake by simply taking my time. At the time of realising my mistake I felt a bit confused and shocked because I didn’t even recognise my mistake where as someone else had to tell me. The surrounding nurses made me feel responsible for my actions which in turn made me feel a bit guilty and unintelligent. Now, that I think over this experience it makes me feel happy that I felt this way because without going through these emotions I would not see the importance of this mistake and the effect it has on my practice, the staff and ultimately my patient’s health.

One positive factor is when I made the mistake I was corrected and since I am a student nurse it is better to learn now then make a mistake in the future as a RN. However, one negative factor was I felt the nurses got a bit annoyed with me when I made this mistake because I felt like I was bothering them as they were busy as well. I am proud of my responsible, mature attitude and the way I handled the feedback from the nurses. I felt like the other RNS handled the situation well because they did not raise their voices with me and their professional attitudes made me see that it is okay to make mistakes and to learn from it especially in this career.
Looking back at this experience there would be a number of things that I would do differently such as manage my time correctly. This can be done by asking for help from my colleagues or write down what I am going to do at each time and when I should finish it. Another strategy is to focus and take my time when charting the observations and if I can write the observations down first then carefully document them. As a future nurse I would double check that I have documented correctly and accurately and I would manage my time more effectively. This will help improve my practice as I should maintain accurate documentation of information which could be used in nursing research (NMBA, 2014) and to improve patient care. Furthermore, as a future nurse I should value the ethical management of information. This requires a nurse to document information that is accurate, non-judgement and relevant to the health care (NMBA, 2014).
